Rental Assistance Programs Six Month Follow-Up Survey

As you may already know, at the request of our funders, we are now required to do programmatic assessment in the form of a six month follow-up survey on all clients who have received assistance through the Homelessness Prevention and Rental Start-Up programs. This process went into effect on July 1, 2005. At this time we are contacting you to request your assistance in obtaining survey information for all clients whose period of assistance ended between July 1 and July 31, 2005. From this point forward, you will be contacted each month regarding clients whose period of assistance ended six months prior.

If you have been contacted about completing the RAP survey for one of your clients, you may fill out and submit the survey online. Please only fill out the survey for the clients we have already contacted you about. By clicking on the link below, you will be re-directed to a secure page. All information submitted will remain confidential. You may fill out and submit the survey as many times as necessary. If you have questions or need assistance filling out the survey, please contact us at (617) 450-1289 or (617) 450-1253.
